S podpisom izjavljam, da želim postati član/-ica DBŠ Black Box in da sprejemam statut društva ter sem se pripravljen/-a ravnati po njem.

Društvu dovoljujem zbiranje, obdelavo in uporabo mojih osebnih podatkov za potrebe delovanja društva, pri čemer je dolžno ravnati v skladu z določili Zakona o varstvu osebnih podatkov (ZVOP-1).

Dovoljujem tudi javno objavljanje slikovnega, video in zvočnega materiala, ki prikazuje dejavnost društva in vsebuje moje posnetke.

Seznanjen/a sem, da udeležba na vadbah in aktivnostih v DBŠ Black Box vključuje tveganja poškodb (udarci, padci, zvini, zlomi, pretresi ipd.).

Zavestno in prostovoljno sprejemam vsa tveganja, povezana z udeležbo na treningih, sparingih in uporabi opreme kluba.

Izjavljam, da sem zdravstveno sposoben/a za športno aktivnost in da bom o morebitnih zdravstvenih omejitvah obvestil/a trenerja.

Obvezujem se, da bom spoštoval/a pravila kluba in navodila trenerja, ter da bom opremo uporabljal/a skrbno in odgovorno.

Strinjam se, da klub in trenerji ne odgovarjajo za poškodbe ali posledice, ki bi nastale pri vadbi, razen v primeru dokazane hude malomarnosti ali namernega povzročanja škode.

Seznanjen/a sem, da klub priporoča sklenitev nezgodnega zavarovanja za primer poškodb.

By signing, I declare that I wish to become a member of DBŠ Black Box, that I accept the association’s statutes and that I am prepared to comply with them.

I permit the association to collect, process and use my personal data for the purposes of the association’s activities, and the association is obliged to act in accordance with the provisions of the Personal Data Protection Act (ZVOP-1).

I also permit the public publication of photographic, video and audio material showing the association’s activities and containing recordings of me.

I acknowledge that participation in training sessions and activities at DBŠ Black Box involves a risk of injury (strikes, falls, sprains, fractures, concussions, etc.).

I knowingly and voluntarily accept the risks associated with participation in training, sparring and the use of club equipment.

I declare that I am medically fit for sporting activity and that I will inform the coach of any health restrictions.

I undertake to follow the club rules and the coach’s instructions and to use the equipment carefully and responsibly.

I agree that the club and its coaches are not liable for injuries or consequences arising during training, except in cases of proven gross negligence or intentional harm.

I acknowledge that the club recommends taking out accident insurance covering possible injuries.
